Doctors’ Letters

You may require a doctor’s letter for various reasons eg fitness to travel or confirmation of medical conditions.  Please contact reception to make your request which will be dealt with in the most appropriate way.

If you are pregnant and require a doctor’s letter as proof of ‘permission to travel’ please be aware that many airlines require your doctor’s letter to be signed within 48 hours of international travel.
Please note that you will not be provided with a doctor’s ‘permission to travel’ letter if you are due in less than 30 days or if your pregnancy is considered high-risk.

Please note the surgery will make a charge for some doctors letters.

HGV and PSV Medicals

The DVLA require a medical on first application, at the age of 45, then every 5 years to 65, then annually. The medical can be performed by any doctor registered or practicing in the United Kingdom or in any other EU/EEA country. The doctor must complete a D4 form which you should bring with you to your appointment.

We can provide HGV/PSV medicals for any person applying for a license. Please call the Practice to book an appointment.

Taxi Medicals

The local Council has the following requirement of all those persons holding or applying for a Hackney Carriage Drivers Licence or a Private Hire Driver’s Licence: A Group 2 medical check is required for all new and renewal private hire and hackney driver applicants. The Group 2 medical checks are applied to all the applicants irrespective of age. The Group 2 medical checks must be carried out by the applicants own General Practitioner (GP) or at the same doctors’ practice with access to the applicant’s full medical history.
